Use semantically correct HTML elements

The multiple choice input choice isn't actually made up of any inputs. Specifically they should be inputs with the type "radio". This would improve screen reader capabilities and keyboard navigation. We have a blind user who is unable to use these forms because there is no feedback when using a screen reader.
